To make your first visit easier, we recommend completing your new patient forms ahead of time. This allows us to review your medical and dental history in advance so your appointment can focus more on you and less on paperwork.
If you are coming from another dental office, you can also complete a records release form. This lets us request your previous dental information, so we have a complete picture of your oral health without you having to track anything down yourself.
Simple, Flexible Ways to Complete Your Forms
You can fill them out online before your appointment, or complete them in the office when you arrive. If you prefer doing them in person, arriving a few minutes early is helpful so we can keep your visit running on time.
